AI Max is the newest piece of automation in Google Ads, and Google is not shy about it. It is switched on by default in every new Search campaign, and Google says advertisers who turn it on "typically see 14% more conversions or conversion value at a similar CPA/ROAS." For a busy dental practice, that sounds like an easy win: flip a switch, get more patients.
It can help. But in the rollout this guide has followed, AI Max comes last, and there is one campaign it never touches at all. Both rules come from the same place. AI Max works by loosening control, and loosened control only pays off in an account that already knows what a good patient looks like.
This chapter explains what AI Max actually does, why it belongs at the end, where it fits in a dental account, and the guardrails to set before you ever switch it on.
AI Max Is a Switch, Not a Campaign
AI Max is not a separate campaign type. It is a set of automation you turn on inside a Search campaign, and Google describes three parts:
- Search term matching. Google uses broad match, plus matching that does not depend on keywords at all, to show your ads on searches beyond the ones you chose. It learns from your keywords, your ads and your website to decide which searches are relevant.
- Text customization. Google uses generative AI to write new headlines and descriptions, drawing on your existing ads, your landing pages and your other assets.
- Final URL expansion. Google sends people to whichever page on your website it predicts will perform best, rather than the page you chose. This one only works if text customization is on.
Put simply, AI Max lets Google decide which searches you show up for, what your ad says and where the patient lands. Each of those is something this guide has spent a chapter teaching you to decide on purpose.
It Comes Last Because It Needs Everything Else First
In practice, AI Max behaves like broad match with even more freedom. And like broad match, it is only as good as what it has to learn from.
Look at what AI Max draws on. It learns which searches matter from your conversion history, which only means something if Chapter 1's tracking is counting real new-patient calls and bookings. It writes ads from your website and landing pages, which only helps if those pages say something specific about your practice, as Chapter 8 covered. It picks landing pages, which only works if you have good pages to pick from. And it needs a negative keyword list that has been built and maintained for months.
That is why, in this rollout, AI Max comes after the Isolation campaign, the Exploration campaign, Performance Max and, for practices with the budget, Demand Gen. By then, the account has a long record of which searches turned into patients, a mature negative list and ads and pages that have been tested. Switching AI Max on in a new account skips all of that and asks Google to guess.
Never in the Isolation Campaign
The Isolation campaign from Chapter 6 exists to protect the dental searches you already know will book: exact match, manual bids, ads written for each search and landing pages built for each service. AI Max undoes every one of those. It widens the matching, rewrites the ads and changes the pages.
It does not even work properly there. Google's own help pages say AI Max's search matching needs automated bidding and does not work with manual bids, which is exactly how the Isolation campaign is bid. That is why Chapter 9 had you switch AI Max off when you built the campaign. It should stay off for as long as the Isolation campaign runs.
Five Questions That Decide Whether You Are Ready
Before you run an AI Max experiment, be honest about whether the account is ready. If the answer to any of these is no, fix that first. AI Max will not fix it for you.
- Is conversion tracking counting real patients? Calls of 30 seconds or more and real booking requests, not page views or form starts.
- Has the Exploration campaign been steady for a couple of months? A cost per conversion that holds near its target, not one still swinging week to week.
- Is the negative keyword list mature? Built before launch and added to every week since.
- Does every service you advertise have its own page? AI Max chooses among your pages. It cannot choose a page that does not exist.
- Is there budget to test without starving what works? The Isolation campaign and brand campaign should stay fully funded while the experiment runs.
If all five are yes, the account has given AI Max something real to learn from. That is the whole reason it comes last.
Where It Belongs: On Top of Exploration, as a Test
There is one campaign in a dental account where AI Max is a natural fit: the Exploration campaign. It already runs on broad match with Maximize Conversions and a target CPA, as Chapter 7 set up. Its whole job is to find searches you could not predict. AI Max is the same idea, taken further.
Even there, do not simply switch it on and hope. Google offers a built-in AI Max experiment that splits an existing Search campaign in two: part of it runs with AI Max off, as the control, and the rest runs with AI Max on. Because it happens inside one campaign, Google says it produces results faster than building a separate test campaign.
Run it that way. Let both halves run long enough to produce a meaningful number of conversions, then compare the thing that matters: the cost of a new patient, checked against the ceiling you worked out in Chapter 7. Google's 14% figure is Google's own number, from its own data. Treat it as a claim to test in your market, not a promise.
There are a few requirements to know. Google says the experiment cannot be set up on a campaign that already has text customization on, or one that uses a shared budget or a portfolio bid strategy. Another reason to keep the Exploration campaign simple.
Set the Guardrails Before You Switch It On
AI Max comes with controls, and for a dental practice they are not optional extras. Set them before the experiment starts.
Keep patients off the wrong pages. URL exclusions stop final URL expansion from sending people to pages that were never meant to receive ad traffic. For most practices, that means excluding the blog, the careers page, the patient portal login, billing and financing forms, and any page for a service you no longer offer.
Fence in what Google can write. Text guidelines let you control the ad text AI Max generates. Google allows up to 25 term exclusions, words or phrases that must never appear, and up to 40 messaging restrictions, plain-language rules about what the ads should and should not say. For a dental practice, that is where you prevent the AI from writing something you cannot stand behind:
- Exclude terms like "free," "cheap," "painless," "guaranteed" and "cheapest," unless one of them is genuinely true and approved for your practice.
- Restrict messaging that promises results, implies a treatment will not hurt, or suggests a discount you do not offer.
- Spell out how the practice name and the dentists' names must be written.
Control brands and places. Brand exclusions keep AI Max from chasing searches for names you do not want, and locations of interest, set at the ad group level, can reflect where patients are searching from.
Dental advertising carries more risk than most when an ad overpromises. A practice is responsible for what its ads say, even when a machine wrote the words. The guardrails are how you keep AI Max's writing inside what you would say yourself.
Read What It Is Doing, Every Week
AI Max is not a set-and-forget feature, and Google gives you the tools to watch it. In the search terms report, searches that came from AI Max show up with their own match type, labeled "AI Max," alongside the headline and landing page that were used. The landing pages report shows which pages AI Max selected.
Check both regularly, the same way Chapter 6 asked you to check the Exploration campaign's search terms. Add negatives for the searches that are not patients. Exclude pages that should not be receiving traffic. Tighten the text guidelines if the ads drift. And keep a close eye on cost per new patient. If AI Max is bringing in more patients at a cost below your ceiling, keep it. If it is bringing in more clicks and more questionable calls, turn it off and keep what the experiment taught you.
Whatever Google Calls It Next Year, the Rules Stay the Same
AI Max is also where Google is moving several older features. As Chapter 9 noted, Dynamic Search Ads and the old campaign-level broad match and automatically created assets settings are being folded into AI Max over 2026 and 2027. The name on the screen may change again. The approach should not: add it last, test it against a control, fence it in with guardrails, and judge it by the cost of a new patient, not by the number of clicks.
